Medication Regimen
The parent or legal guardian of any pupil taking medication on a regular basis must inform the school nurse or [other contact person] of the medication being taken, the current dosage, and the name of the supervising physician. With the consent of the parent or legal guardian, the school nurse may communicate with the physician and may counsel with the school personnel regarding the possible effects of the medication on the pupil.
Administration of Prescribed Medication for Pupils
Any student who is required to take medication during the school day must submit the appropriate medical documentation to the school. Documentation may be provided through either our Medical Forms Package or a Physician's Orders printout.
The required documentation must include the following:
- The name of the medication
- Dosage
- Route of administration
- Time(s) the medication should be administered
- The reason for the medication
- Common side effects (if any)
- Start and stop dates for the medication
All medical forms must include the medical office stamp for verification. Additionally, please ensure the forms are signed in the designated areas. Medications must be delivered to the school office in their original container or package, with the prescription pharmacy label clearly visible.
The student will be assisted by the school nurse or other designated trained personnel in administering the medication according to the physician's instructions.
Please note that self-carrying medication is not recommended for elementary school students. Medications should be stored and administered by designated school personnel to ensure student safety and compliance with prescribed instructions.
